Medical Records Organization & Healthcare Summary
Helping veterans and families make sense of complex healthcare records.
- Field Grade Advisory Group provides administrative medical-record organization for veterans, caregivers, and families managing extensive or fragmented healthcare information.
Under the day-to-day operational leadership of Brooke Hossfeld, MPH, CIC, MLS(ASCP)CM, our operating team organizes the records clients provide, creates neutral healthcare chronologies, and makes complicated information easier to locate and understand.
This service is intended for healthcare organization and personal planning. It does not include medical opinions, legal advice, or assistance preparing or pursuing a VA benefits claim.

WHO THIS SERVICE IS FOR
This service may be helpful for:

WHAT WE CAN DO
01
Organize and inventory records
Depending on the selected scope, our operating team may inventory and label the records provided, arrange records in chronological order, and create a neutral timeline of documented appointments, diagnoses, procedures, and treatment.
02
Compile key healthcare information
We can compile provider, facility, medication, and procedure lists, and identify duplicate documents, missing date ranges, and incomplete record categories.
03
Clarify information in plain language
We translate administrative and medical terminology into plain-language organizational notes and develop a general list of questions for clients to discuss with licensed healthcare professionals.
04
Prepare an organized record package
We prepare an organized personal-record package for healthcare coordination and long-term planning. Our summaries reflect information already documented in the records. We do not diagnose conditions, determine causation, or decide what information should be used in a VA claim.
WHAT YOU MAY RECEIVE
Depending on the scope of the engagement, clients may receive an indexed record inventory and a chronological healthcare timeline.
You may also receive provider and facility directories, and medication, procedure, or diagnosis lists based on the supplied records.
We can identify missing or incomplete record categories and provide general questions to discuss with healthcare providers.
You receive an organized personal copy of the reviewed records. These materials are designed for personal organization, healthcare discussions, and care coordination—not as a claims strategy or VA evidence packet.
HOW THE SERVICE WORKS
Our records-organization service follows a clear, step-by-step process.
1. Define the Organizational Need
We begin by identifying the healthcare, caregiver, or personal-planning purpose for the review. Claim eligibility and appeal strategy are outside the scope of this service.
2. Securely Provide Relevant Records
Clients provide the healthcare records needed for the agreed organizational task through our secure upload process. Please do not provide unrelated sensitive documents.
3. Records Are Organized
Our operating team inventories, labels, and arranges the supplied information. We document what the records say without offering medical conclusions or selecting information to support a benefits claim.
4. Receive the Organized Materials
Clients receive the agreed chronology, index, lists, or organizational summary for personal reference and healthcare coordination.
5. Consult the Appropriate Professional
Medical questions should be discussed with a licensed healthcare professional. Questions involving a particular VA claim, rating decision, effective date, appeal, eligibility determination, or evidence strategy should be directed to a VA-accredited representative.
